To make a copyright application, you will need to download the Power of Attorney (Copyright) here.

Submit the completed and signed Power of Attorney, together with:

• 15 samples of the work or its substitute
  
• A copy of the assignment document, if the applicant is not the author

Original copies of the completed and signed forms must also be mailed to our office, for submission at a later date. Please send all forms to:
