INDUSTRIAL DESIGN

To proceed with your industrial design applications, please submit a copy of the following forms:

• Power of Attorney Design
• Declaration of Ownership Design
• Deed of Assignment (if the applicant is different from the designer)
• Deed of Assignment Priority Rights (for applications filed under the International Convention)

Submit the above forms together with a drawing and a brief description of your design.

Original copies of the completed and signed forms must also be mailed to our office, for submission at a later date. Please send all forms to:
